Sciatica

Sciatica Treatment in Sydney | Rose Bay Chiropractor

Sciatica is one of the most misdiagnosed and mismanaged conditions I encounter in clinical practice. Patients regularly arrive having been told their MRI is “fine”, or that their scan shows “mild” changes that couldn’t possibly explain their symptoms. But the Gonstead analysis frequently tells a different story. When I examine posture, assess the disc levels at L4-L5 and L5-S1 under weight-bearing X-ray, and perform specific neurological testing, the picture becomes clearer. A small disc bulge on a lying-down MRI can look very different when that same person is standing and loading their spine.

If you’ve experienced a sharp, burning, or shooting pain that travels from your lower back down through your buttock and into your leg, you may be dealing with sciatica. It can be debilitating, making it difficult to sit, stand, walk, or sleep comfortably.

What Is Sciatica?

Sciatica describes irritation or compression of the sciatic nerve. Common causes include disc bulge or herniation at L4-L5 or L5-S1, degenerative disc disease, piriformis syndrome, and spinal stenosis. The specific location and nature of your symptoms gives important information about which nerve root is involved.

Why the No-Twist Approach Matters for Sciatica

For people with acute sciatica, particularly those with disc involvement, rotational spinal manipulation can sometimes worsen symptoms. The Gonstead System uses precise, controlled adjustments directed at the specific spinal level involved, without placing rotational stress on the disc. By using bulk-billed 3D EOS Digital X-ray alongside postural analysis and specific motion palpation, I can confirm exactly which level is affected and deliver care accordingly.

How Exercise Physiology Supports Sciatica Recovery

Once the acute phase settles, rehabilitation is essential to prevent recurrence. As an ESSA-accredited exercise physiologist, I develop personalised programs to restore normal movement patterns, strengthen the core and gluteal muscles that support the lower back, and gradually reintroduce loading appropriate to your lifestyle.

What to Expect at Your First Visit

Your first appointment begins with a thorough case history and physical examination. I will assess your neurological function, spinal mobility, and posture. Where relevant, digital X-rays will be taken and reviewed with you in the room. You will receive a clear explanation of what is contributing to your symptoms and a recommended care plan before any treatment begins.

Sciatica often overlaps with back pain and lumbar spine conditions. Our exercise physiology program supports your return to full function once the acute phase settles.
